Step 1

The initial contact is made with our office at 4 Recreation Road, Sydenham, London SE26 4ST

At this point the referring Social Worker will be made aware of Krossroads referral criteria and policies, the current position regarding present and future vacancies, and services offered by the project.

If accepted, the Social Worker will then need to inform Krossroads in writing giving all relevant details of the placement. This will primarily be via the projects ’Referral Form and young person’s Questionnaire. These will need to be faxed to the office, and a decision will be made soon afterwards.

A meeting will then need to be arranged with the Social Worker, Young person and relevant staff at Krossroads. The meeting will be expected to cover the following areas, although this should not be viewed as a definitive agenda.

  • The accommodation offered
  • The amount of support required for the young person
  • Areas of work required to be undertaken with the appointed Key Worker
  • Education/Training
  • Employment Prospects
  • Health Issues
  • Life Skills

Within this meeting, the boundaries and expectations of the service provision and living within a Krossroads property will be explained to the young person and their Social Worker. Practical and financial arrangements will be discussed separately with the Local Authority.

Step 2

Moving in and financial arrangements

Once the offer of accommodation and support is accepted, the placement and financial agreements will be sent for signing by the manager of the Social Work team. No placement can progress until these documents have been agreed, signed by the appropriate person and received by krossroads (a faxed copy will suffice in the first instance), but original hard copies must be received within five working days of the placement being made.
